Meet David J. Patrishkoff

Connecting operational excellence, high-reliability systems, and human-governed AI.
David J. Patrishkoff is President of E3—Extreme Enterprise Efficiency LLC, the original consulting and operational-excellence company, and Founder of E3 AI Systems, its human-governed AI and decision-intelligence brand.​ He is a Lean Six Sigma Master Black Belt, former senior defense analyst, author, and adjunct professor whose work focuses on human–AI decision quality, operational excellence, and high-reliability systems.

Across more than three decades, he has advised leaders and improved systems in more than 60 industries, held C-level engineering, quality, and manufacturing roles, trained or mentored more than 3,000 Lean Six Sigma professionals, facilitated high-reliability organization training for more than 23,000 healthcare employees, and built more than 200 AI applications and prototypes. His government work includes cascading-risk war games and decision-support consulting for Pentagon organizations, including authorized work involving secure generative AI services in government environments.

His current research and prototype work centers on Human–AI Decision Filters and Wisdom Loops—a distrust-first decision architecture for qualifying evidence, challenging assumptions, bounding authorization, monitoring action in reality, and carrying validated lessons forward.
